- This is an annual fee dental savings plan that allows individuals and families to receive quality dental care from Mountain West Dental for a percentage off the office’s usual fees.
- No physical membership card will be issued as this plan is only available in our dental office.
- Membership enrollment fees must be PAID prior to any dental care procedures.
- All payment for services is due in full the time of service in order to receive the discount savings.
- If the participant does not utilize their savings plan benefits, plan is NON-REFUNDABLE and offers no additional discounts.
- Membership is for one year from enrollment date.
- Membership will NOT automatically renew; member will be responsible for renewing each year.
- Payment for additional dental services are the member’s responsibility.
- Fees for dental services may change at any time.
- It is the member’s responsibility to schedule and keep all appointments offered as part of the Health Smiles Plan. Please notify the office at least 48 hours in advance if you need to change an appointment to avoid cancellation fees.

Dental Exclusions & Limitations:
- This dental discount plan is not insurance and cannot be combined with any other dental insurance or discount.
- This plan is non-transferable, family members cannot be substituted in for another member.
- If referred to a specialist, they will not honor this discount.
- An orthodontic patient must remain as a plan member for the duration of treatment to retain discount plan benefits.
- Services for injuries or conditions which are covered under Worker’s Compensation or Employer’s Liability laws.
- Services which are covered under automobile or medical insurance.
- Services which are without cost to the member by any municipality, county or other political subdivision.
